DSI Exchange - All Countries Except U.S. and Europe

Please note that prior to processing your exchanges, DSI requires the following items:

1. A completed Exchange form.
2. A formal paper copy of the completed purchase order.
   (Please see your Purchasing department.)
3. Implants being returned.

All must be included in the return shipment. Any items missing may result in a delay of your implant exchange return shipment.
