Continuous Glucose Telemetry Award Application

- A description of your current and pending research (if applicable), highlighting its relevance to the intent of the award (up to 3 pages).
- Information about how the equipment and supplies will be used in your proposed study study (up to 1 page).
- A Biosketch or CV (up to 4 pages).
Applications are no longer being accepted as the deadline has passed.
Application deadline is May 1, 2018.
Award notification will be performed on or before June 22, 2018.
